Insurance
Please bring your most current insurance card with you to every visit. (Most insurance providers issue new cards annually.) If you do not have your insurance card with you at the time of your appointment, we kindly ask that you bring it to your next visit so we can update our records accordingly.
We currently accept all insurance plans; however, it is the patient’s responsibility to verify out-of-network coverage and benefits with their insurance company.
Please note that insurance plans frequently change, with new policies, procedures, provisions, and restrictions. For the most accurate and up-to-date information regarding your coverage, we recommend contacting your insurance company directly. You can find their contact number on the back of your insurance card.
All self-pay patients are required to pay at the time of the appointment.

Referrals
If your insurance plan requires a referral, please bring a copy of the approved referral to your visit. It is your responsibility to ensure the referral reaches our office prior to your appointment. If a referral is not provided, you may be asked to sign a waiver.
