Appointments

We offer two easy ways to schedule your appointment:

  • Book Online: The quickest and most convenient option! Use our Zocdoc integration by clicking the “Book Online” tab on the right side of your screen. You’ll see real-time availability and can schedule your visit anytime—day or night.
  • Call Us: If you prefer to speak with someone, call 631-968-7700 and select Option 3 to reach our scheduling team.

Same-day appointments are always available for urgent sick visits. If you’re feeling unwell, please let us know so we can see you promptly.

New Patients

If you’re visiting us for the first time, please arrive a few minutes early and bring the following:

  • Your insurance card
  • A valid photo ID
  • A current list of medications

What to Expect

A medical assistant will begin your visit by performing a brief triage, which includes:

  • Taking your vitals
  • Reviewing your medical history
  • Discussing the reason for your visit

You will then be seen by one of our medical providers to complete your visit.

Before You Leave

Before exiting the office, please stop at the front desk to:

  • Schedule any follow-up appointments as recommended by your provider
  • Arrange any additional testing or referrals that were discussed during your visit

Taking care of this before you leave ensures a smooth, coordinated care experience.

After-Hours Emergency Care

If you are experiencing a life-threatening emergency, please call 911 immediately or go to the nearest emergency room.

For urgent medical concerns that cannot wait until normal office hours, you can reach our 24/7 answering service at 631-455-4177.

Please note: This line is for urgent issues only.

For all other inquiries—including scheduling, prescriptions, or test results—please call our office during regular business hours.

Hospital Follow-Up

If you’ve been discharged from the hospital or a nursing home, it is critical that you follow up with your primary care provider within 3 days, but no later than 7 days after your discharge.
Why is this Follow-Up Important?
Following up with your provider shortly after discharge allows us to:

  • Monitor your recovery and check for any complications
  • Review your medications to ensure they are appropriate for your ongoing care
  • Address any questions or concerns you may have about your discharge instructions or next steps
  • Adjust treatment plans if necessary to improve your health outcomes

Confidentiality

Your privacy is extremely important to us. All medical records are handled with the highest level of confidentiality and in full compliance with HIPAA regulations.
Our staff is fully trained and strictly bound by confidentiality policies as a condition of their employment. Your medical information will only be released with your written authorization.
You will be asked to review and sign our privacy policy during your visit.
